The ABI/MFP Program Supervisor is responsible for overseeing the House Manager, staffing and operations of multiple group homes for adults with acquired brain injuries and other disabilities. The ABI/MFP Program Supervisor will train and supervise staff members. Ensure that the homes are safe, clean and comfortable at all times. Makes certain that individuals are given all needed supports to help them live as independently as possible, maintain optimal health and are given sufficient opportunities to engage in their desired activities and interests at home and in the community.
Provide and maintain a safe and warm home environment for individuals in the program. Oversee housekeeping and ensure program is organized and accessible.
Provide training and supervision to Homelife Counselors, and Relief staff. Meet regularly with each staff member for supervision and complete annual evaluations/Assessments as required. Follow through on employee disciplinary action.
Spend a minimum of 10 hours a week during program hours in the assigned residence(s). Review status of individuals, staffing, housing concerns, etc. Support assigned individuals at medical appointments as needed.
Review and approve all bi-weekly timesheets, time off requests, mileage sheets and grocery request. Review and approve all individuals spending requests.
Ensure that staffing requirements/schedule are accurate and complete for each house assigned.
Coordinate ISP process with DDS Service Coordinators for assigned caseload. Ensure that all assessments are completed 10 days prior to the ISP meeting and submitted on time and that Provider Support Strategies (PSS) are completed 15 days after the ISP meeting is held. Complete pre-ISP checklist and quarterly summaries. Attend all ISP’s for assigned caseload.
Ensures that all staff/ are trained in knowing how to implement and document PSS and behavioral interventions.
Collects behavior intervention data and submits to clinical department as required. Also collect and summarizes the PSS implementation data on a quarterly basis and submits as required.
Manage crisis situations in conjunction with the Residential Director and follow emergency procedures.
Assure timely reporting and complete documentation of significant incidents, etc., in accordance with DDS regulations.
Identifies staff training needs and communicates them to the Director of ABI/MFP Residential Services or other appropriate personnel.
Submit report and meet regularly with Supervisor; Attend meetings and trainings as assigned.
Participate in rotating schedule for 24-hour administrative on-call support for individuals and staff in residential services; maintain related documentation. Follow all on-call procedures.
Explore opportunities for individuals to achieve community membership and make new connections through networking, and thus identifying people in the community as potential “bridge builders” between the people we support and the community.
|High school diploma or GED with 4 years working with individuals with disabilities, 1 year in a supervisory capacity. Ability to complete all initial and ongoing background checks including but not limited to: CORI, SORI, Fingerprinting, LEIE, Driving record, E-verify, Physical, PPD and licensing and/or certification checks (i.e. nursing). Valid US driver’s license and ready access to a vehicle.|
