Patient Care Coordinator (40 hours/week)
Salary Range: $38.00 - 47.97 per hour, based on experience (Weekly Pay)
This is a 40-hour Patient Care Coordinator position with Pioneer Valley Hospice & Palliative Care (PVHPC). The Patient Care Coordinator is a Registered Nurse who provides direct supervision and oversight for field staff and is responsible for case assignments and serves as a clinical resource and educator for its nursing team. This position is responsible for ensuring that patient care is coordinated and managed appropriately by supporting nursing services, along with coordinating with other clinical departments to ensure an interdisciplinary approach is maintained.

Essential Job Functions
- Demonstrate advocacy for those served by the agency and promote the agency's mission/services.
- Performs joint visits with all nursing staff periodically.
- Reviews all admission documentation, diagnosis codes, discharge/revocations, deaths, and care plans for accuracy.
- Evaluates the team’s participation in risk, assisting and attending as necessary.
- Daily reports for synchronization productivity, visit frequency, and unverified visits.
- Collaborate with the Clinical Director for annual review, coaching, and mentoring.
- Communicates with the appropriate HCIB administrative assistant regarding insurance and billing for referrals.
- Assists in assuring that there are sufficient clinical personnel during hours of operation.
- Responsible for assisting clinicians by reviewing and evaluating cases.
- Collaborate with the clinical team and are responsible for instructing and guiding clinical staff to promote more effective performance and delivery of quality hospice services.
- Conducts regular record reviews and communicates findings and recommendations to the Clinical Director and hospice personnel.
- Responsible for complying with accepted professional standards and principles.
- Assists in the planning and implementation of quality control programs.
- Responsible for reviewing available patient information related to the case, including home visits to determine hospice care and eligibility.
- Responsible for coordinating and leading IDT meetings.
- Responsible for conferencing with the Clinical Supervisor and the Clinical Director regarding any questions about an individual’s eligibility for hospice services.
- As necessary, fills in for the Referrals and Admissions Coordinator duties, such as, but not limited to:
- Administrator on-call responsibilities as needed.

Qualifications
(Minimum qualifications will be considered required unless specifically stated otherwise)
Experience:
- Minimum of 2-4 years of experience in health care and management preferred.
- Experience in hospice care preferred.
- Understanding of hospice philosophy and issues of death/dying.
Education and Training:
(Acquired through formal education, outside study, training on jobs of lesser degree, or by any combination of these. May be expressed in terms of formal educational equivalents):
- An associate's degree in nursing is required; a bachelor's degree in nursing is preferred.
License, Certification & Registration:
- Possesses and maintains current CPR Certification.
- Current MA licensure: RN
- Valid driver’s license
Other Requirements:
- Demonstrates good verbal and written communication and organization skills. • Ability to deal tactfully with personnel, patients, families, and community.
- Knowledge of corporate business management, governmental regulations, CHAP standards, local and state resources, assistance programs, and Medicare/Medicaid preferred.
- Demonstrates autonomy, organization, assertiveness, flexibility, and cooperation in performing job responsibilities.
- Understands principles of pain/symptom management.
- Ability to problem-solve, delegate responsibility and encourage high-level performance from Nursing Team.

Working Conditions
- Uniform / Dress Requirements (see policy). Workspace primarily in the Greenfield office, which is a busy, professional office with a moderate level of noise, activity and interaction with others. Most of the time is spent on visits to patient/family homes where space, climate, facilities, and conditions may vary.
- May be exposed to extremes of heat and cold in all weather conditions. Must drive in various weather conditions on roads in varying repair.
- May be exposed to infections and contagious diseases. Contact with patients under a wide variety of circumstances. May be exposed to patient elements. Subject to varying and unpredictable situations. Handles emergencies or crises.
- Travel required.
- OSHA exposure category:
- Category I: Position includes tasks that involve exposure to blood, body fluids, and tissues.
- Category II: position includes tasks that involve no exposure to blood, body fluids, tissues but employment may require unplanned Category I tasks.
- Category III: position includes tasks that involve no exposure to blood, body fluids, or tissues; would not be required to perform Category I tasks.
