    Position Summary:

    This is a full-time, 40-hour per week position. 

    Salary Range: $115,000-125,000 per year

    The Patient Care Manager (PCM) reports to the Executive Director and is responsible for ensuring that patient care is coordinated and managed appropriately by providing direct supervision of Clinical services, along with coordination of other departments to ensure an interdisciplinary approach to care is maintained. The position requires exceptional communication and clinical skills, a demonstrated ability to manage professional staff, and a comfort level with end-of-life care.

    Essential Job Functions

    • Coordinates and oversees all direct and indirect patient services provided by the clinical personnel.
    • Demonstrates an understanding of hospice philosophy and principles, along with the ability to clearly and effectively communicate these to others.
    • Reviews and triages referred patients’ medical diagnoses, prognoses, and clinical courses to assess eligibility in collaboration with the Medical Director and hospice team.
    • Participates in the insurance verification and authorization processes as needed.
    • Provides support for clinical care and electronic documentation to hospice team clinicians, both in the office and in the field, to ensure that the plan of care is completed, documented, and implemented in a timely and appropriate manner.
    • Administrative and clinical supervision of the clinical team, including supervisory visits and written performance reviews for clinical staff.
    • Maintains expertise in case management responsibilities per regulations and agency standards to assist case managers as needed.
    • Maintains an adequate and appropriate inventory of supplies and equipment for the provision of patient services.
    • Facilitates weekly Interdisciplinary Team meetings in collaboration with the Patient Care coordinators and Executive Director.
    • Participates in staff orientations and training, quality improvement meetings and activities, and annual policy reviews.
    • Oversees the schedule for nursing services, ensuring that sufficient clinical personnel are available during operating hours.
    • Leads the training and supervision of daily tasks, conducts ride-along visits, and carries out written performance reviews.
    • Participates in the Administrator On-call rotation.
    • Maintains thorough knowledge of hospice regulations and standards and be responsible for complying with and promoting accepted professional standards and principles.
    • Reviews documentation from staff for completeness and provide feedback and assistance as necessary.
    • Demonstrates excellent and respectful communication skills.
    • Works harmoniously and cooperatively within the team structure, willingly assisting coworkers and consistently maintaining productivity while demonstrating high standards of professionalism.
    • Prioritizes the rights of patients, family members, and coworkers.
    • Ensures that IDT productivity expectations align with the position.
    • Interprets operational indicators to detect census changes and increases or decreases in volume that could impact staffing levels, revenues, and expenses
    • Perform other duties as assigned.

    Qualifications

                (Minimum qualifications will be considered required unless specifically stated otherwise)

    Experience: 

    1. Minimum of 4-5 years of experience in health care.
    2. Clinical experience in hospice, home care, or Long-Term Care is required.
    3. Leadership experience in a healthcare setting preferred.
    4. Understanding of hospice philosophy and issues of death/dying.

    Education and Training: 

    (Acquired through formal education, outside study, training on jobs of lesser degree, or by any combination of these. May be expressed in terms of formal educational equivalents): 

    • Associate’s degree in nursing is required.
    • Bachelor's degree in nursing is preferred.

    License, Certification & Registration:

    • Possesses and maintains current CPR Certification.
    • Current MA licensure: RN
    • Valid driver’s license

    Other Requirements:

    • Demonstrates the ability to multitask, is detail-oriented with strong time-management skills
    • Has creative problem-solving skills.

    Working Conditions

    • Uniform / Dress Requirements (see policy). Workspace primarily in the Greenfield office, which is a busy, professional office with a moderate level of noise, activity and interaction with others. Most of the time is spent on visits to patient/family homes where space, climate, facilities, and conditions may vary.
    • May be exposed to extremes of heat and cold in all weather conditions. Must drive in various weather conditions on roads in varying repair.
    • May be exposed to infections and contagious diseases. Contact with patients under a wide variety of circumstances. May be exposed to patient elements. Subject to varying and unpredictable situations. Handles emergencies or crises.
    • Travel required.
    • OSHA exposure category:
      • Category I: Position includes tasks that involve exposure to blood, body fluids, and tissues.
      • Category II: position includes tasks that involve no exposure to blood, body fluids, tissues but employment may require unplanned Category I tasks.
      • Category III: position includes tasks that involve no exposure to blood, body fluids, or tissues; would not be required to perform Category I tasks.
