PHYSICAL THERAPIST - PER DIEM Salary Range: $52–$68/hour This is a per diem position.The physical therapist is responsible for planning and administering physical therapy treatment appropriate to the hospice patient This position offers significant autonomy, including the ability to structure your own schedule, while receiving support from a compassionate, skilled team. Travel is required throughout Pioneer Valley to provide care in a variety of settings—patients’ homes, assisted living residences, nursing facilities, or hospitals. Essential Job Functions - Provides skilled physical therapy evaluations and treatment in accordance with physician orders.
- Promptly and thoroughly documents and maintains clinical and administrative records regarding patient limitations/interventions, treatments, responses, and progress to administered therapy.
- Assess and provide patient and family/caregiver education and information pertinent to diagnosis and self-plan of care.
- Participates in the coordination of hospice services, appropriately reporting the identified needs for hospice interdisciplinary group members.
- Participates in personal and professional growth and development.
- Participates in the organization’s quality management program as appropriate.
- Attends staff meetings as appropriate.
- Attends IDT meetings as appropriate.
- Other duties as assigned.

Qualifications (Minimum qualifications will be considered required unless specifically stated otherwise) Experience: - Minimum of two (2) year's experience in health care.
- Experience in a hospice care preferred.
- Understands hospice philosophy, and issues of death/dying.
Education and Training: (Acquired through formal education, outside study, training on jobs of lesser degree, or by any combination of these. May be expressed in terms of formal educational equivalents): - Graduate of an accredited Physical therapy program
License, Certification & Registration: - Possesses and maintains current CPR Certification.
- Current Massachusetts licensure: PT
- Valid driver’s license
Other Requirements: - Demonstrates good verbal and written communication, and organization skills.
- Strong observation skills and problem solving skills.

Working Conditions: - May be exposed to extremes of heat and cold in all weather conditions. Must drive in various weather conditions on roads in varying repair.
- May be exposed to infections and contagious diseases. Contact with patients under a wide variety of circumstances. May be exposed to/occasionally exposed to patient elements. Subject to varying and unpredictable situations. Handles emergency or crisis situations. Travel required. OSHA exposure category:
- OSHA exposure category: Category I
- Category I ? Position includes tasks that involve exposure to blood, body fluids, tissues.
- Category II ?Position includes tasks that involve no exposure to blood, body fluids, tissues, but employment may require unplanned Category I tasks.
- Category III ? Position includes tasks that involve no exposure to blood, body fluids, tissues; would not be required to perform Category I tasks.
