Description:
Position Highlights:
UCP is hiring! We are looking for a Spanish/English Intake and Service Navigation Coordinator to act as the first point of contact and meet with each prospective individual/family to promote UCP Services, obtain all required required documentation and make referrals to appropriate department(s) demonstrating exceptional customer service skills.

Other duties for this role will include but are not limited to attending IEP and ISP meetings and using the goals and guidelines outlined in the ISP/IEP to address the individual’s needs. maintaining intake records, case notes, services and contacts, developing partnerships with educational, state and other non-profit human service organizations, conducting quality assurance audits, compiling data and preparing reports, maintaining current policy and procedure manuals and assisting staff with program and service preparation where needed.
Requirements:
Qualifications:
Bachelor’s degree in related human services field and a minimum of two years positive work experience in a human service agency ideally providing program services.
Fluent Spanish and English oral and written skills
Applicant must be available to work flexible hours when needed
Must be able to work from home
Maintain valid driver's license, current auto insurance, availability of a private vehicle.
Must be able to drive frequently to conduct in person services
Comfortable using remote applications such as Zoom
Comprehensive data entry skills
Proficiency with Microsoft Office 365
Highly organized
Strong customer service acumen to positively represent UCP and provide a conscientious and caring attitude
Must be willing and able to perform essential job functions through in-person, face to face contact as required, in accordance with health and safety guidelines.
