Description:
Case Manager-Full Time
LifePath seeks a full time case manager. Ideal candidate will be comfortable working independently and as part of an interdisciplinary team providing case management and administrative support for the Home Care program. The position serves consumers in Franklin county and the North Quabbin area.

Bachelor degree in relevant field and two years experience providing case management support to older adults and/or persons with disabilities. Must have a commitment to supporting people to live safely and independently in the community. Knowledge and experience in person-centered planning a plus. Successful candidate will have valid driver’s license, reliable transportation and ability to frequently travel throughout assigned areas. Use of personal cell phone, data and downloading of company provided applications required.
