Description:

Full-time 40 Hours

Some Flexibility in Scheduling 
Springfield/Westfield Area

Responsibilities:

Unique position combining case management elements with direct care tasks
Work in collaboration with an RN on record keeping and collecting documentation for  medication administration in several group home settings (50%)
Collaborate with RN on direct medical supports for 6 individuals with disabilities who live together in one home (50%)
Perform routine medical treatment as needed or prescribed, i.e. temperatures, blood pressures, etc.
Consult with program staff on specific medical interventions, training, techniques and documentation requirements.
Make referrals with RN for appropriate health care and dental needs if applicable.

 

 

Requirements:

Licensed Practical Nurse with at least 1 year experience.
Valid driver’s license & Insured Vehicle
