Case Manager; Geriatric/Long Term Services and Support - Full Time


The Geriatric Services and Support Coordinator (GSSC) /Long Term Services and Support case manager (LTSC) work in the Senior Care Options (SCO - ages 65+) and One Care (ages 21-64) programs, respectively. This person is responsible for functioning as a member of the Integrated Care Team. The position provides evaluation of the need and eligibility of members for support and services in the community while acting as an advocate, as needed. The GSSC/LTSC promotes independent living and total wellness working with people with disabilities and elder populations. The position assists elders and individuals with disabilities to identify and attain personal goals necessary to promote good health and enhance their ability to live independently in the community. Frequent driving throughout the assigned geographical area is required. The caseload is in the Wachusett region.

This position requires the ability to work remotely. The ideal candidate will have excellent interpersonal skills, be able to work well independently, and as part of a collaborative team. Computer literacy, ability to multitask, and strong organizational skills required. LSW license or Bachelor’s degree in a relevant field required with 2 years experience. A valid driver’s license and reliable transportation is required. Required to use personal data plan & to download and use company paid applications on your personal cell phone.
