Become a part of a team of competent and compassionate professionals as an Outreach Counselor in the Community Support Program (CSP) at Behavioral Health Network! Outreach Counselors use cultural competence, knowledge of community resources such as detox facilities, homeless shelters, and courts, and have a willingness to treat challenging and diverse populations. They provide advocacy services for individuals with behavioral health, substance abuse, and medical care difficulties around the Western Massachusetts community. They create service plans with measurable goals and timelines for completion and assist individuals with implementing these plans and enrolling in qualified programs.

Outreach Counselors collaborate with all appropriate members of the individual’s multidisciplinary team in order to meet service plan goals and documents phone calls, consultations, visits, and other activities timely and effectively. They provide mobile respite services, seeking out and transporting individuals to various locations and services they need. Finally, Outreach Counselors reach out refer specialty care managers when needed. In this position, Outreach Counselor are surrounded by chances to learn and grow with trainings, supervision and support, and professional advancement opportunities. This Outreach Counselor position is part of an initiative funded through the Department of Mental Health (DMH).

The Community Support Program (CSP) is a short-term program designed to help adults with mental health or substance use challenges to live independently in the community. BHN CSP serves adults who have recently been in or who are at risk for psychiatric hospitalization or admission to detox, providing access to community resources such as transportation, housing stability, and day programs along with others.

Requirements:
Outreach Counselors in BHN’s Community Support Program have the following qualities:
• Bachelor’s degree in a mental health related field (required).
• Experience working with individuals with behavioral health and/or substance use issues.
• Knowledge of mental illness, the interface between mental health and medical conditions and systems, and substance abuse and care management.
• Familiarity with Internet, word processing, and email systems.
• Bilingual in English and Spanish (preferred).
• MAP certification (preferred).
• Ability to serve as a part of an on-call rotation.
• Valid driver’s license and reliable transportation (required).
