Join our team of competent and caring professionals as a Nurse Care Manager at Behavioral Health Network’s One Care program! Through their organized nature and attention to detail, Nurse Care Managers use their problem-solving skills to identify the needs of individuals and discover solutions for the healthcare team to meet those needs. The Nurse Care Manager uses their broad knowledge of sound clinical theories and principles to provide high quality-clinical care management services.

The Nurse Care Manager is team-based, making sure that all appropriate members in the Integrated Care Team are included in decision-making when they implement and review comprehensive, person-centered Integrated Care Plans. Nurse Care Managers ensure positive health outcomes and prudent use of resources by monitoring the effectiveness of Integrated Care Plans and adjusting, as needed. Collaborating and consulting with internal and external service providers, the Nurse Care Manager meets individuals' needs timely and efficiently while maintaining direct service productivity.

Requirements:
Nurse Care Managers at Behavioral Health Network have the following qualities:
• Knowledge of mental illness, the interface between mental health and medical conditions and systems, substance abuse, and care management.
• Familiarity with internet, word processing and emails systems.
• RN License (required).
• Associate’s Degree in Nursing (required).

Substance Abuse Testing: positions embedded with these specific healthcare organizations are subject to a pre-employment substance abuse test, which includes the testing for the consumption of THC (Marijuana)
