(HCHC) seeks a Behavioral Health Care Coordinator for a mission-driven non-profit devoted to providing medical, dental, behavioral health, eye care services, and community programs to everyone, regardless of ability to pay. The candidate will effectively coordinate all Behavioral Health Services for all HCHC sites. The candidate will serve as a hub and point of contact for all patient referrals, and provide case management support as needed in referring to outside agencies as well as inter-agency resources. This position reports to the directly to the BH director. Must be well organized and experienced in providing administrative support in a Behavioral Health setting, and work well in a team. Preference given to candidates with Bachelor’s degree in social work, counseling, human services field, or relevant experience.
