Become a part of our team of competent and compassionate professionals as the After Care Coordinator in the Integration program at Behavioral Health Network! The After Care Coordinator provides high-quality, community-based care to individuals served through care coordination, case management, and direct supporting services to address their medical, substance use, and behavioral health needs. They assess the needs of program participants prior to their discharge and coordinates with nursing staff to provide medications needed for their re-entry. Working with community MAT providers and other resources, the After Care Coordinator coordinate services to ensure continuity of care and help individuals with accessing resources.

The After Care Coordinator regularly communicates with the individuals and coordinates re-entry appointments with medical, behavioral health, and recovery support providers, offering comprehensive case management and direct assistance to address social determinant needs, such as food stamps, housing, transportation, etc. Working closely with the individual, their supervisor, site Program Director, and community providers, the After Care Coordinator develops trusting, respectful relationships and works as part of the team in collateral providers meetings. Finally, they may provide transportation to individuals served to community follow-up appointments, and they complete all necessary paperwork in a timely manner.

The Integration program at BHN holds each individual at the core of what it does. Through the coordination and integration of care, One Care assists adults ages 21-64 with medical, behavioral health, or substance abuse concerns in navigating the health care system and improving their health outcomes.

Requirements:
The After Care Coordinator at Behavioral Health Network has the following qualities:

Bachelor’s degree in a related field (preferred). May be substituted with relevant experience and Associates degree, CHW or Recovery Coach Certification, or High School Diploma.
Bilingual in Spanish and English (preferred).
Knowledge of mental illness, the interface between mental health, medical, substance abuse and care management.
Familiarity with Internet, word processing and emails systems (required).
Professional demeanor and phone skills in communicating with members and collaborating providers.
Cultural competence in working with participants, with a variety of cultural backgrounds.
Experience in providing assistance with obtaining resources and/or navigating systems to individuals with SUD and behavioral health problems.
Experience advocating for individuals who have been incarcerated.
Good communication skills, interest in helping others.
Ability to work effectively as a team and/or individually.
Meets contractor criteria required by HCHC/CODAC that may include extensive background investigation, drug testing and medical clearance.
Valid driver’s license, and active automobile and insurance (required).
