Become a part of our team of competent and compassionate professionals as a Community Health Worker (CHW) in the Certified Community and Behavioral Health (CCBHC) program at Behavioral Health Network! Community Health Workers provide targeted care management and skill building, particularly for clients who have a history of being unsuccessful engaging in clinic-based treatments. CHWs may be recovery coaches, certified peer specialists, or family partners. These staff will conduct the Arizona Matrix with clients who have not recently completed the assessment in another program. CHWs will pilot technology care enhancements including telehealth and My Strengths patient self-management resources. The CCBHC program is person-centered, evidence-based, and focused on collaborative goal setting to address unmet medical, behavioral mental and social needs. Requirements: Community Health Workers at Behavioral Health Network have the following qualities: •High school diploma; Bachelor’s degree preferred. •1-2 years of work experience in a related mental or behavioral health field (required). •Knowledge of mental illness, the interface between mental health and medical conditions and systems, substance abuse, and care management (required). •Bilingual English and Spanish (required). •Being able to do Outreach-responding to an individual’s needs in community settings (required) •Ability to work well in a team setting (required). •Display emphatic skills and the ability to establish rapport with individuals served, family members, and service providers (required). •Familiarity with Internet, email systems, and electronic health records (EHRs) (required) •Professional demeanor and phone skills in working with persons served, team members, and community collaterals (required).
