This individual will coordinate all areas of treatment planning with the treatment team in addition to carrying a caseload. Responsibilities include contacting and communicating with family, workers and team members, working in collaboration with the Program Clinician regarding treatment plan writing, and facilitating parent and peer support groups. Other duties include reviewing all intake packets, conducting interviews, and completing pre-admission evaluations. This position requires on-call support to the programs. Qualifications of the Clinical Care Coordinator include the following: • Must be a Master’s Level Clinician • Active and valid driver’s license and reliable personal vehicle • Must provide verification of adequate personal auto insurance on the vehicle(s) to must be used, including, at minimum, Optional Bodily Injury to Others (coverage part 5 on a Mass Auto policy) limits of $100,000 per person/$300,000 per accident, and Damage to Someone Else's Property (part 4) limits of at least $100,000. *This must be brought to the interview. • Ability to read, analyze and interpret client information, reports, clinical documents, education plans and the like.
