Help individuals with disabilities live independently in the local community!

Join our team of competent and compassionate professionals as a Case Manager in the Independent and Community Living Supports program at Behavioral Health Network! Case Managers provide direct and indirect services to individuals in their homes, community, and work settings to support their growth, independence and safety! They also provide supervision and complete administrative duties to ensure the well-being of the individuals served.

Funded by Department of Developmental Services (DDS), Massachusetts Rehabilitation Commission, United Way and Masshealth the Individual & Community Living Supports program provides hourly supports to adults with disabilities living on their own or with their family in the community. This program also provides assistance to adults living in Nursing Facilities to transition home. The ultimate goal is to help the individual live in a home of their choosing, with supports that enhance the quality of their life, maintain their health and safety and allow them to be as productive/engaged in their family or community as their skills allow them to be.

Requirements:
Case Managers at Behavioral Health Network have the following qualities:

Individuals must be committed to consumer empowerment and growth, human rights, and community integration.
Must have familiarity with internet, word processing and email systems.
Requires a minimum of two years experience in human services.
Training is provided by BHN.
Minimum of Associates Degree in a related field required.
Must have valid driver’s license, and a reliable car available for working hours.
