Responsibilities include to provide coaching and support for assigned clients, accompanying the youth’s family to meetings on behavioral health treatment needs, engaging with clients in the home and in community activities, working one on one with youth and monitoring progress on the attainment of treatment plan goals and objectives, face to face and phone care coordination with collateral providers. To be qualified, applicants must have a High School Diploma and a minimum of two years of relevant direct service experience working with youth and families who require behavior management to address mental health needs. Approved CORI, DCF and driving background checks are required and a reliable vehicle for driving clients is also required.
