Description:
Guidewire, Inc., a human service agency, is looking for an extraordinary person, currently residing in the Springfield, MA area, to share their home, and fill an important role as a Shared Living Professional. This living arrangement is arranged as an independent contractor opportunity and is expected to begin in the fall of 2018 and continue ongoing through contract terms and agreement.

Requirements:
- Available bedroom in your residence (must be located in Springfield, MA or surrounding area)
- At least 18 years of age
- Ability to safely transport the person in your care, as needed
- Acceptable Massachusetts and Federal background check for any household member and or other person 15 years of age or older regularly on the premises.
- Maintain certifications such as CPR, First Aid, and OSHA, which are required by DDS Regulations and offered regularly at our agency)
Responsibilities:
Daily living support for a person with disability, positive attitude, be a good role model, demonstrate positive life choices, and participate in community outings or events with the person supported, welcome them into your world (family, friends, etc), and share life experiences, have patience, compassion, ability to educate, communicate professionally, be punctual, document medication(s), ability to work in a team environment, and work cooperatively to achieve goals.
