Description:
Elder Services of Berkshire County Inc. (ESBCI) is hiring for the full time position of Geriatric Support Service Coordinator (GSSC)/ Long Term Services and Supports Coordinator (LTSSC). This position is an integral part of the Primary Care/Interdisciplinary Team for case management of individuals enrolled in both Senior Care Options (SCO) and One Care plans. The GSSC/LTSSC will provide initial and ongoing assessments of these clients and their needs, including eligibility for relevant services, public benefits and community resources. In this position, you will need to develop and coordinate service plans based on these assessments in a client-centered, culturally sensitive model and ensure the clients’ needs are met by services. This position requires the ability to work independently, multitask, conduct home visits and caregiver/provider meetings, make frequent telephone contact, and meet regular documentation deadlines. Strong interpersonal skills are crucial to develop and maintain constructive and collaborative relationships with clients, caregivers and supports, colleagues at ESBCI and SCO/One Care representatives, as well as service providers, vendors, and other community agencies. Requirements: Applicant must have a minimum of a bachelor’s degree and two years of professional experience with individuals with disabilities and those over the age of 65, with at least one year involving work in a setting where persons over 65 receive health-care services (e.g. hospital, nursing facility, community health agency, or interdisciplinary program within an ASAP). Bi-lingual English/ Spanish preferred. Applicants must have the ability to work remotely, if and when required, with reliable internet connection, telephone, and transportation.
Requirements:
Minimum of bachelor’s degree and two years professional experience with individuals over the age of 65, with at least one year involving work in a setting where persons over 65 receive health-care services (e.g. hospital, nursing facility, community health agency, or interdisciplinary program within an ASAP). Applicant should have the ability to work remotely as needed with reliable internet connection, telephone, and transportation.
