Care Manager - Adult Foster Care (AFC)
Job Type: Full-time (37.5 hours)
Job Location: Hybrid remote, caseload throughout the Pioneer Valley+ per hour
The Adult Foster Care (AFC) Manager is a professional support position responsible for planning, coordinating, and monitoring AFC members. The care manager’s responsibilities include providing psychosocial and counseling assistance for the AFC program and timely reporting.
Care Manager Responsibilities:
- Conducting initial and ongoing assessments of the AFC qualified setting.
- Obtaining a social history and conducting a psychosocial assessment and evaluation for each AFC member.
- Select, train, evaluate and supervise AFC caregivers in conjunction with RN or LPN.
- Participate in the development, implementation, and ongoing review of the AFC plan of care for each member.
- Conduct on-site visits and associated progress notes with each member at the qualified setting bi-monthly (alternating with the bi-monthly visit by the registered nurse) for Level 1, and monthly for Level II.
- Review monthly the AFC Caregiver logs.
- Make referrals to appropriate service providers if a member requires health or social services other than AFC.
