| LPN - The Center for Human Development (CHD) is currently seeking a part-time LPN for its Adult Day Health programs focused on elder care located in West Springfield. In addition to monitoring and assessing participants, medication is administered and collaboration with participant’s medical team is essential. LPN's are involved in creating; monitoring and ensuring care plan effectiveness. LPNs are a key member of the team in educating participants and their families to maintain the member’s independence. LPN's must not only manage participant care but help oversee the CNA staff. | |

| LPN License required as well as ability to pass a Criminal Offender Record Information check (CORI). Must have a valid Driver’s license and reliable vehicle for work purposes. One year of experience working with the elderly, people with disabilities, behavioral challenges, and cognitive disabilities needed. Computer literacy required.
