Salary Range: Location: 15 Per Hour SPRINGFIELD MA Job Type: Category: Full Time Human Services Education Level: Work Experience: Associate Degree Preference: Required 2+ Preference: Required Skills/Certifications/Licences: Description: Position: Full-Time Rehabilitation Specialist Location: Springfield, MA Rate of Pay: $17.25 per hour Hours: 40 hours per week Schedule: 8:00 am-4:30 pm, Monday-Friday (hours may vary based on programmatic need) Full-Time Rehabilitation Specialist Summary: Responsible for gathering data, documenting and presenting each member’s goals and objectives in accordance with ISP regulations; coordinating services with case managers and parent/guardians of program member’s goals and objectives, issues, concerns and abilities; and ensuring the provision of high quality services. The Rehabilitation Specialist is also responsible for overseeing the screening and prioritizing of referrals; documenting member progress; implementing behavior management strategies; investigating incidents and all other duties as assigned. Full-Time Rehabilitation Specialist Qualifications: Associate’s degree required in Human Services or related field, Bachelor’s degree preferred. Two years’ experience participating in the ISP process including documentation, report writing and support strategies. Two years of experience with individuals with developmental disabilities. Requirements: Proficient in Microsoft Office (including Excel and Word). Experience in Credible and HICSIS preferred. Must be organized, detail-oriented, have solid time management skills and be able to work in a fast-paced multi-tasking environment. Demonstrated ability to deliver high level of customer service/satisfaction with both internal and external customers. Excellent communication, interpersonal and listening skills. Must have a valid driver's license, 3 years driving experience, good driving record, use of vehicle and willingness to transport participants in personal and/or agency vehicle.
