Valley Medical Group is seeking a personable and reliable LPN to work at our Easthampton Health Center! Hours: This is a 32 hour a week position. Hours range from 7:30am to 6:00pm. The candidate would be asked to work an occasional Saturday or Sunday as part of our Urgent Care Rotation. Position: LPN Location: Easthampton, MA Responsibilities: Reporting directly to the RN Team Leader, the Licensed Practical Nurse is responsible for providing nursing care to patients using clinical skills and procedures within the limits of the Nurse Practice Act. As the LPN you will manage all types of messages (phone calls; PORTAL messages, patient cases) and will respond to all within 24 hours and document all interactions in the EMR. The Licensed Practical Nurse will also communicate effectively with providers in reviewing daily appointments and to maintain patient flow. Duties also include preparation of patients for the office visit by getting accurate vital signs, taking a history, doing medication reconciliation and reviewing allergies.
