Join our team of competent and compassionate professionals as a Community Health Worker in the Project FIT (Families in Transition) program at Behavioral Health Network! As core members of the team, Community Health Workers focus on engaging families and supporting family members in accessing needed services in the community. They deliver services to family members as outlined in their unique care plans, working closely with the Clinical Care Manager.
The goal of Project FIT (Families in Transition) is to help families in need achieve a greater level of health, wellness, stability and autonomy. Project FIT coordinates services to meet their medical and behavioral health needs, along with ensuring that the family locates and maintains stable housing. The FIT team is assigned based on the family’s needs and consists of a Master’s level Clinical Care Manager and either a Community Health Worker, Recovery Coach or Family Partner. Services are provided in settings that are comfortable for the family.

Requirements:
Community Health Workers at Behavioral Health Network have the following qualities:
• Highly organized, flexible, and team-oriented.
• Attentive to details.
• Driven to solve problems.
• Knowledge of community and resources
• Family work.
• Bachelor’s degree or comparable work or lived experience (preferred).
• Bilingual in Spanish and English (preferred).
• Valid driver's license and a reliable car available for working hours (required).
