Become part of our team of competent and compassionate professionals as a Bilingual Complex Care Clinician in Behavioral Health Network's Care Management Team, caring for individuals with complex health concerns in a community health center setting! The Care Management Team partners with insurers and integrated primary care practices to provide whole health management. The Team provides high-quality community-based care to maintain high-quality outcomes throughout measurement and interpretation of collected data. Clinicians in the complex care team connect with members at the PCP office, their home, or other community setting for face-to-face and phone contact to improve an enrollee's health status and care. They conduct biopsychosocial comprehensive assessments and deliver evidence based brief interventions. With their teams, Clinicians create and implement care plans to address identified needs, remove barriers, and make needed referrals toward improving the health of the enrollee. Finally, Clinicians provide education and self-management support to individuals and participate in the supervision of staff.

The Care Management Team at BHN has extensive experience in managing and coordinating care for individual needs across the healthcare spectrum that includes case management in our Departments of Mental Health and Developmental Services programs, insurance provider contracts, and Substance Abuse and Mental Health Services Administration (SAMHSA) grants. We offer care management for adults with intellectual disabilities, mental health diagnoses, forensically-involved adults with mental health diagnoses, substance use disorders, or co-occurring disorders. BHN also offers care management for adults and children with chronic medical concerns.

Requirements:
Bilingual Complex Care Clinicians in Behavioral Health Network's Care Management program have the following qualities:
• Bilingual in Spanish and English (required).
• RN, LICSW, LCSW, or LMHC (required).
• A current Massachusetts license for a discipline listed above.
• Four or more years experience in social or behavioral health.
• 3-5 years of experience in providing direct services to individuals with behavioral health conditions.
• Exceptional written and oral communication skills.
• High energy, a strong work ethic, integrity, is kind and empathetic, has a sense of humor, and enjoys working with a collaborative, team oriented system.
