Position: Community Outreach Nurse Part-Time
Location: Holyoke, MA
Rate of Pay: $26.50 per hour
Hours: 20 hours per week
Schedule: Monday-Friday, 8am-5pm (schedule varies based on programmatic need)

Part-Time Community Outreach Nurse Summary: The Community Outreach Nurse is responsible for overall management of the medical aspects of the AFC and MassHealth programs. The nurse is responsible for administration of the medical/nursing components of the AFC program. S/he is responsible for ensuring programmatic and contractual obligations are met.

Part-Time Community Outreach Nurse Qualifications: Associate’s degree with R.N. license and five years related experience OR Bachelor’s degree with R.N. license and three years related experience. One year of supervisory or human service experience is desired. Knowledge of and respect for divers populations.

Part-Time Community Outreach Nurse Requirements: Ability to use computers to include Microsoft Word, data entry, email, and internet essential. Valid driver’s license, 3 years driving experience, good driving record, use of vehicle with compulsory auto insurance and vehicle registration, and willingness to transport program members in personal and/or agency vehicle. Must be able to travel within the region to access residential settings, employers, and community organizations.
