This individual will coordinate all areas of treatment planning with the treatment team in addition to carrying a caseload. Responsibilities include planning and scheduling meetings associated with overall treatment and medical appointments, contact and communicate with family, workers and team members, and work in collaboration with the nurse to assist with medical care, treatment and reviews. Other duties include facilitating parent, peer support and independent living skills groups and attendance and participation in milieu and staff meetings. Qualifications of the Clinical Care Coordinator include the following: • Must be a Master’s Level Clinician • Active and valid driver’s license and reliable personal vehicle • Must provide verification of adequate personal auto insurance on the vehicle(s) to be used, including, at minimum, Optional Bodily Injury to Others (coverage part 5 on a Mass Auto policy) limits of $100,000 per person/$300,000 per accident, and Damage to Someone Else's Property (part 4) limits of at least $100,000. *This must be brought to the interview. • Ability to read, analyze and interpret client information, reports, clinical documents, education plans and the like.
