If this describes you, you may be a great ICC Clinical Care Coordinator in the Intensive Care Coordination program at Behavioral Health Network! Serving youth with serious emotional or behavioral health needs in the Springfield, Westfield, and Ware, MA areas. ICC Clinical Care Coordinators collaborate and coordinate services with state agencies, the school system, primary care, and other providers on the behalf of the children/youth and families. Additionally, ICC Clinical Care Coordinators develop and facilitate Care Planning Teams, ensure medical case consultations are requested, and perform CANS and safety assessments. ICC Clinical Care Coordinators have a lot of opportunities for learning and growth, as well! Individuals in this position are committed to doing what it takes to support children with behavioral health challenges and their families. They will become fluent in the wrap around model of intervention with youth and families and will be able to become a trained facilitator of the model. There will also be the opportunity for professional development through DPD and to become trained in several evidence-based treatment models.

The Intensive Care Coordination (ICC) program at Behavioral Health Network is a voluntary outreach service that uses ‘wraparound’ principles to guide the work for youth with serious emotional or behavioral health needs. Wraparound is simply the coordination of multiple services via a tailored plan to address the needs of the youth. More research about the wraparound model can be found at http://nwi.pdx.edu/ .

Requirements:
ICC Clinical Care Coordinators have the following qualities:
• A Master's Degree in a mental health related field, must be licensed, or license-eligible and actively working towards licensure.
• CANS-MA Certification.
• Conduct comprehensive assessments inclusive of the CANS-MA; conducts multi-axial diagnostic assessments and Serious Emotional Disturbance determination for referred children and youth.
• Thrives in a fast paced work environment.
• Engaging, creative, resourceful and flexible.
• Organized, energetic, and enjoys working in different settings within the community.
