Responsibilities include planning, scheduling, and implementation of activities, outings and groups that offer the opportunity for community integration, effective communication and participation with members of a multidisciplinary team of medical and clinical staff, completion of required paperwork, and development of supportive and nurturing relationships with participants and their family members and caregivers. Requirements: Bachelor’s Degree in social work or human services field strongly preferred, experience in an adult day health setting or work within an activities program, demonstrated ability to lead groups and organize activities, commitment to empowering adults and elders to live the most dignified and meaningful lives.
