Directly supervise clinical staff. Responsible for establishing and maintaining working relationships with area home health agencies, health care facilities and skilled nursing facilities. Monitor and provide clinical support for the MassHealth Frail Elder Waiver Program, Enhanced Community Options Program and Community Choices Program. Directly supervise the ASAP RN staff and the CSSM Care Advisor. RN, BSN or AD with a minimum of 2 years experience in a community or long term care setting. Supervisory experience required.
