LPN - The Center for Human Development (CHD) is currently seeking a part-time LPN for its Adult Day Health programs focused on elder care located in West Springfield.

Adult Day Health’s LPNs are a critical part of the team. In addition to monitoring and assessing participants, medication is administered and collaboration with participant’s medical team is essential. LPN's are involved in creating; monitoring and ensuring care plan effectiveness. LPNs are a key member of the team in educating participants and their families to maintain the member’s independence. LPN's must not only manage participant care but help oversee the CNA staff.
Requirements:
LPN License required as well as ability to pass a Criminal Offender Record Information check (CORI). Must have a valid Driver’s license and reliable vehicle for work purposes. One year of experience working with the elderly, people with disabilities, behavioral challenges, and cognitive disabilities needed. Computer literacy required.
