We are looking for a 32 hour Diabetes Nurse Educator to be part of our growing Amherst and Greenfield centers! Position: Registered Nurse, CDE or Registered Dietitian, CDE Location: Amherst & Greenfield, MA Responsibilities: As the Diabetes Nurse Educator you will work as part of an interdisciplinary team that includes, e.g., physicians, nutritionists and case management. Under the supervision of the Diabetes Education Program Director, the successful candidate will deliver diabetes disease management services, education and direct care to VMG patients with diabetes. The Diabetes Nurse Educator will work closely with our Endocrinology specialists and Family Practice providers, offering services and education in 1:1 appointments, diabetes education classes, as well as participation in specific Shared Medical office visits facilitated by VMG providers. Additional responsibilities: Insulin Pump education and training, Continuous Glucose Monitors and Insulin Titration. Qualifications: Valid Massachusetts Nursing license or valid Dietitian registration and Massachusetts license Certified Diabetes Educator (CDE) OR eligible to become a CDE (Nurses with strong diabetes background are encouraged to apply- will train an appropriate candidate Experience in an outpatient setting Proficient use of personal computers in a Windows based environment.
