Valley Medical Group is seeking a full time Nutritionist for our Northampton and Easthampton Health Centers. The candidate will work as part of an interdisciplinary team that includes a wide range of members of the health care team (ex: Physicians, Diabetes Nurse Educator, Case Management). Under the supervision of the Diabetes Education Program Director, the successful candidate will deliver Disease Management services and education to Valley Medical Group (VMG) patients with diabetes and other diagnostic conditions. Medical Nutrition Therapy is offered in 1:1 appointments, Diabetes Education Classes, as well as through participation in specific Shared Medical office visits facilitated by VMG providers. The Nutritionist will work closely with Family Practice providers and Endocrinology specialists.
