Description:
The Hospice RN Case Manager is a skilled professional member of the Interdisciplinary Team, responsible for implementing the nursing component of the plan of care and coordination of other clinical and support services. The position also provides direction and supervision to hospice aides and serves as the primary liaison with patients’ physicians and ancillary health providers.

RN Case Manager provides direct nursing care to hospice patients, supports their families, and coordinates other clinical and support services as included in the hospice plan of care. Specifically, develops, implements, and manages the plan of care from referral through discharge for hospice patients and families per regulations and agency standards. Also coordinates on-going provisions of patient care and facilitates timely communication of adjustments to Plan of Care among interdisciplinary team members.

Serves as primary liaison with patients' physician and ancillary health providers in the community.

First shift 32 and 40 hr/wk positions available, including occasional weekend, evening, holiday and night on-call rotation required as needed to meet programs ability to provide 24-hour services to patients..
Requirements:
•Nursing degree from accredited school of nursing
•Current MA RN licensure
•Minimum of two years recent direct care nursing experience, preferably in palliative, hospice, oncology, med/surg, long term or home health
•Knowledge and understanding of hospice philosophy and approach to care
•Excellent communication skills and ability to deal professionally with a wide range of individuals and situations
•Proficiency with computer documentation skills
•Reliable transportation and valid driver's license
•Current CPR Certification
•Effective communication in person, in writing, on the telephone and via electronic medical record
